The member may change his name. The change in name in the records of the Institute will be noted under the following circumstances.

In Case of Female Members

Application for change in name, alongwith a copy of marriage invitation card,
Or
Attested copy of Marriage Registration Certificate
Or
An affidavit duly sworn in before a First Class Magistrate/Notary Public as the case may be.

In Case of other Members

Attested copy of Gazette Notification or Original copy of advertisement in a leading Newspaper for change of name,
Or
Attested copy of affidavit, for the change in name, duly sworn in before a First Class Magistrate/Notary public as the case may be.

The Institute will issue a letter of change in name to the member on satisfying the above requirements.
